Measure and record vital signs.Family Support & Communication: Serve as a comforting, professional presence, reporting any clinical changes or urgent needs promptly to the hospice nursing supervisor.Environment Maintenance: Assist with maintaining a clean, calm, and supportive home environment for the patient and their visiting loved ones.Technology Proficiency: Demonstrated ability to navigate MatrixCare efficiently with your cellphone, ensuring timely and accurate documentation of patient care, timesheets, and travel time.Ability to Travel (when needed): Capable of independent travel to various local client residences, when needed, and maintaining detailed and precise records of travel time and destinations.RequirementsMust possess an active Pennsylvania CNA certificationMinimum 6 months of hospice, home health, skilled-nursing, or related clinical experienceHighly punctual, reliable, and trustworthyA deeply caring, empathetic, and resilient attitude suited for home hospice environmentsA reliable vehicle and Driver's License Are you ready to take control of your schedule while providing meaningful, one-on-one hospice care?
